Care and Wellbeing Service: Home Care

Our new Care and Wellbeing Services (CWBS) will replace our current home care services from Monday 3 June 2024. 

The new service will be delivered by 14 care providers from 16 dedicated Patches across the city. It is aimed at delivering the best possible service experience and outcomes for people in receipt of home care services.

The key principles of the new service are:

Workforce Development

The workforce has been placed front and centre of the CWBS developments. Adult Care and Wellbeing  will collaborate to support learning in key areas such as:

  • advanced dementia care
  • falls prevention
  • Technology Enabled Care
  • digital skills

We will also be working closely with the providers to build on current career pathways with the aim of supporting retention and recruitment of the highly valued care workforce.

Strength based approach

This is a collective strength-based approach. It is done through:

  • assessments
  • trusted reviews
  • care delivery

It supports people’s potential for independent living, improving quality of life and optimising care costs.

Outcomes

A move away from time and task to a more flexible and responsive outcome-based services. Our aim is to continually improve the services that we deliver to people in receipt of care.

Technology Enable Care and Digital Services 

Digitally enabled services are at the core of service delivery. 14 providers have a digital care planning capability, which provides smarter, more intuitive ways of delivering care. This will allow care staff to have more time to care. The new contract will ensure that providers collaborate in the developments to embed Technology Enable Care (TEC) services. These are aimed at supporting:

  • independent living
  • right sizing care packages
  • helping to enable proactive and preventative care, which delivers the best possible outcomes for people

Climate

Support of the response to Sheffield’s climate emergency. The compact geographical patches will significantly reduce care workers travel time between visits and allow for the introduction of new care workers operating on foot. This approach will help build capacity, and support care delivery during periods of extreme weather.

This is to deliver a high-quality sustainable market.

Adult Social Care Strategy: living the life you want to live

These are services that contribute to the delivery of our strategy. They enable people to live independently, be connected to their community and supported by the resources around them. 

Strong customer voice

This ensures that people in receipt of care, their families and carers can positively contribute to the continual improvement of services that matter to them. 

Longevity 

We are prioritising both the continuity of services and care through a 7-year contract, complemented by optimised sustainable contract sizes. There is an option to extend by a further 3 years. Optimised sustainable contract sizes.

Collaboration

This is consistent with our core value of ‘together we get things done.' We have a focus on neighbourhood working, and we have moved from provider competition to collaboration as we strive to deliver outstanding care across the city. Providers are now a part of our senior management team family. 

Empowerment

Through flexible and responsive services, such as trusted reviewers (care plans), providers are empowered to deliver independence. There will be more ability to respond timely to changes in need and circumstances.
