Child Disability Register
The Childrens Act of 1989 requires us to hold a Child Disability Register (CDR).
We register all children and young people who:
- are aged 0-25,
- live in Sheffield, and
- have any disability, or life-altering medical condition
The CDR supports linking all the registers and lists held by Sheffield services such as:
- Health,
- Education, and
- Social Services in Sheffield.
We use anonymous information held on the CDR to plan future services for children with disabilities in Sheffield.
Enrolling your child onto the Register
Parents and carers can enrol their child onto the Register.
To enrol, complete the on-line form. We will keep your details on a confidential computer database.

Please re-enrol on the Register to amend your details
What happens next
The Sheffield Special Educational Needs Disability Information and Support Service (SSENDIAS) will:
- contact you via email, and
- send you their latest newsletter within 7 days
